True To You Outreach helps system impacted individuals stabilize during difficult transitions, preventing crises from escalating into emergency responses and reducing recidivism.
Your donation supports early intervention services for homeless adults, adults aging out of foster care, and justice-involved adults during high-risk transition periods. Without timely support, the first weeks following release, displacement, or aging out carry a significantly higher risk of homelessness and crisis.

We serve individuals across Cumberland and surrounding counties in North Carolina, focusing on stabilization during the most critical moments of transition.
Our work centers on supporting individuals who are working to regain direction, stability, and momentum during periods of uncertainty or disruption. Through non-clinical, person-centered support, we help remove barriers to housing, resources, and basic needs while reducing delays that often contribute to emergency system involvement or recidivism.
